Based on observation, interview, and record review, the licensee did not comply with the section cited above. At 12:30 PM during document reviews, and interviewed, LPA observed the last firedrills was conduct on 8/25/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will conduct a disaster drill firldrills as soon as possible and will submit documentation of the drill to LPA by due date.

Based on observation,interview, record review, the licensee did not comply with the section cited above. At 12:30pm during staff file reiviewed and interviewed, 2 staffs out of 3 staff members does not have their mandated reporter training certificate expired 1/4/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states, she will submit the updated/current Mandated Reporter training certificate for Staff #2 (S2) and staff #3 (S3) to LPA by the due date.

The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review. At 2:00 PM LPAs reviewed staffs file, 2 out of 3 staff were missing Immunization Record. The licensee did not comply with the section cited above in, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, They will obtain a copy of Immnization record for S2 and S3. Licensee will submit the copy to the Licensing office by the due date.
