Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision..(1)....Supervision shall include visual observation. This requirement is not met evidenced by: Based on LPA interviews, 7 out of 10 interviewed staff confirmed that S1 was being on their phone 50-100% of the time while S1 was providing care and supervision for the children in the classroom/playground, which posed an immediate risk to the health, safety and personal rights to the children in care.

This requirement is not met evidenced by: Based on LPA interviews and record reviews, all 4 staff members, who were present on the playground, did not intervene to redirect/stop C1 from causing injury to C2, which posed an immediatel risk to the health, safety and personal rights to the children in care.

This requirement is not met evidenced by: Based on interviews and records review, teacher S1 and aid S2 were supervising 17 preschool children in the Twos classroom; teacher S4 and aid S3 were supervising 17 preschool children in the Older Twos classroom, which posed an immediatel risks to the health, safety and personal rights to the children in care.

This reuirement is not met evidenced by: Based of interviews and records review, S2 and S3 did not meet ECE qualifications to assist qualified teacher to supervise 17 preschool children in the Twos and the Older Twos classroom, which posed potential risks to the health, safety and personal rights to the children in care.

.3 Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, This requirement is not met evidenced by: Based on LPA observations on 05/15/24, 2 teachers were observed supervising 26 children in the Older Twos classroom on 05/22/24 and on 06/05/23, 1 staff supervising 16 children(13 napping, 3 awake including 1 child sitting at the table) in classroom Threes, which posed an immediate risks to the health, safety and personal rights to the children in care.

This requirement is not met evidenced by: Based on LPA interview and record review, Director confirmed that facility was out of compliance with the license's terms by providing care for 3 children over the age of 6 years old, which posed an immediatel risk to the health, safety and personal rights to the children in care.
