Based on observation, interview, and record review, The licensee did not comply with the section cited above in. At 9:00 AM, LPA observed, the Licensee failed to ensure that Staff 1 (S1) did not have the current mandated reporter training certificate, which had expired on 7/15/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated will send proof of Mandated Reporter Training to LPA by the due date.
Based on, observation, interview, record review. the licensee did not comply with the section cited above. At 9:00 AM during document reviews, and interviewed Staff #2 does not have current immunization record (immunization record) MMR, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated will submit to LPA for staff #2 the proof of immunization records before the due date.
