DREAM-I IN BUENA PARK

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Based on LPA’s observation, record review and interview, the staff Guzman Jessica, who has been employed at the facility since 8/8/023, did not have a criminal record clearance. This poses an immediate risk to the health and safety of children in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 5 children's files, Child #3 did not have Physician's Report LIC 701 on file, which posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above, LPAs observed gate on the left of the back playground, has a broken latch; Director confirmed the defect, which poses a potential health, safety or personal rights risk to persons in care.

(d) The surface of the outdoor activity space shall be maintained: (1) In a safe condition for the activities planned.

Based on observation and interview, the licensee did not comply with the section cited above, at 10:08am, LPAs observed animal feces on the grass area of the front playground when teachers are bringing children in care into the playgroud for outdoor activities; Director said staff had checked the playground at 8am this morning and confirmed the hazards and cleaned it up, which posed a potential health, safety or personal rights risk to persons in care.
