Based on record review, the licensee did not comply with the section cited above in two out of two staff files missing Mandated Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to send two copies of Mandated Reporter Certificates to LPA by 8/18/24. Send copies of Certificates to tammy.dutra@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above in C1 and C6 did not have a copy of LIC 995 in childrens files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will get completed LIC995 signed and send to LPA by 8/18/24. Send copies to LPA at tammy.dutra@dss.ca.gov.
(c) The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement is not met as evidenced by: expired CPR/First Aid
