Licensee and assistant last completed mandated reporter training 4/26/23 which poses a potential risk to the health and safety of children in care.
Resolution: Licensee and assistant shall complete the required mandated reporter training at www.mandatedreporterca.com and provide proof of completion to CCL by 3/11/26
Proof of immunizations for licensee's assistant were not available during the inspection which poses a potential risk to the health and safety of children in care.
Resolution: Licensee shall provide proof of immunizations for pertussis, measles and influenza for her assistant. Licensee shall also provide proof of TB clearance for assistant.
C4 did not have Individual Sleeping Plan in file during the inspection which poses a potential risk to the health and safety of children in care.
Resolution: Licensee shall ensure all infants under 12 months of age have an Individual Sleep Plan in their file. Licensee shall provide a copy of this plan for C4 to CCL by 3/11/26.
The licensee did not comply with the section cited above. During inspection there were 4 infants and two preschool aged children present, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall ensure that she remains in compliance with ratio/capacity of the license. Licensee shall submit a letter to CCL by 4/8/22 ensuring that she understands the ratio/capacity of the license and will remain in compliance.
Licensee did not comply with the section cited above. Licensee's CPR/First Aid training expired 2/2/21 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit proof of registration for CPR/First aid training by 4/22/22. Licensee shall follow up with proof that the training as been completed.
