Based on observation and record review, the licensee did not comply with the section cited above in LPA observed 3 expired medications which poses/posed a potential health, safety or personal rights risk to persons in care.

Alterations to Existing Buildings or New Facilities (a) Prior to construction or alterations, the licensee shall notify the Department of the proposed change(s). Based on LPA's observation and LPA's conversation with Director M. Mote it was determined that construction has been going on since 12/26/2023 and the facility failed to inform the department which poses a potential risk to children in care.
