Resolution: Licensee states Adult #1 will obtain criminal record clearance by 11/12/2024. Licensee will send the Department a copy of the completed LIC 9163 by email by 11/12/2024.

Resolution: Licensee states she will conduct and document Emergency Disaster drill and submit to the Department by email by 11/29/24.

Resolution: Licensee states she will document on a sleep log every 15 minutes when infants are asleep and send the department a copy of the sleep log by email by 11/12/24.
