Based on record review, the licensee did not comply with the section cited above in not providing a current disaster drill log, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct a fire drill and submit a copy of the documented drill by 7/11/25 by email. Licensee stated she will post the drills on her board as a reminder to conduct them every 6 months.
Based on record review, the licensee did not comply with the section cited above in not providing the deparement with a current mandated reporter certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will take the mandated reporter training and submit a copy to the department by email by 7/11/25. Licensee stated she will put the dates on calendar as a reminder to complete the training every two years as required.
Based on record review, the licensee did not comply with the section cited above in not providing the department proof of documenting sleep for infants every 15 minutes, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she does check on sleeping infants at all times but has misplaced previous documentations. Licensee stated she will begin documentation today and will xxxxx. LPA reviewed safe sleep resources and left an example of a safe sleep log with Licensee.
