Based on record review and interview, the licensee did not comply with the section cited above in that the Director's file was not maintained at the child care center or available to review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview, observation, and record review, the licensee did not comply with the section cited above in that the licensee changed which rooms were being used for licensed child care, and did not inform the Department of this change, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in that 15 children were present on the playground at the same time. There were only 5 children signed in for the day. All of the children were in the same age group. However, both the school district children and the licensed preschool children were using the playground at the same time, which poses/posed a potential health, safety or personal rights risk to persons in care.

However, the mandated reporter training did not satisfy Assembly Bill (AB) 1207 which poses/posed a potential health, safety or personal rights risk to persons in care.
