Based on observation, interview, record review, the licensee did not comply with the section cited above in completing fingerprint clearances and association in guardian for four employees (DOJ, CAI, FBI) which poses an immediate health, safety or personal rights risk to persons in care. For facility to continue operations, all four employees must complete live scan.

Based on record review, the licensee did not comply with the section cited above. Physican Reports (LIC701) were missing in some of the children's files, which poses/posed a potential health, safety or personal rights risk to persons in care.

Facility will provide filter water purchased from store to ensure children have drinking water until lead testing results are completed and approved by LPA Ali.
