Based on observation , the licensee did not comply with the section cited above because LPA Caro observed 2 baby walkers in the facility which the licensee stated were recently provided as a gift this poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee removed both baby walkers from the facility and stated she is getting rid of them. POC Cleared by visit.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above because the Licensee had 5 infants in care during inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall send one infant home to come into compliance as of 12/18/23, licensee shall also submit a plan to show how she will maintain compliance due to ratio standards.
Based on observation, the licensee did not comply with the section cited above as a baby walker, and baby jumper were observed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall review the requirenment and remove the baby walker and baby jumper form the home.
Based on record review, the licensee did not comply with the section cited above as the Licensee had a Manadated reporter certificate that was expired which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee shall review the regulation and submit a copy of the current certificate by January 9, 2023.
