Director Natanya Moore arrived at the facility 15 minutes after the arrival of LPAs. Initial sampling for Little Elephant Montessori was completed on 3/6/2025 which resulted in an exceedance of 53 ppb in one (1) outlet (Outlet A) which exceeded the Action Level established by the State for exposure. The facility submitted test results to Community Care Licensing during today’s inspection with LIC 9275 and LIC 9276 along with an updated facility sketch of location of the outlet. The facility received the results on 3/24/2025 and replaced the faucet the next day. Director Moore stated the facility is currently not using Outlet A for drinking water and is utilizing bottled water instead. During today's visit, LPAs conducted an inspection and toured the premises with the Head Teacher Larwood. LPAs observed Outlet A with attached hose in the backyard with signage indicating positive lead testing in compliance with AB2370. Director Moore stated that Outlet A is used exclusively used with the hose to water outdoor plants and wet sand in the backyard sandbox playspace. During the visit, Director Moore covered Outlet A with plastic and affixed a "Do Not Consume" sign. Director Moore also contacted the testing facility in order to determine if a flush is required and to schedule retesting. Due to the children potentially being exposed to lead, which is a risk to their health and safety, a Deficiency is cited on page 809D. LPAs discussed a Plan of Correction and facility will submit the documentation for the post-testing requirement. Exit interview conducted and report was reviewed with Director Moore. A Notice of Site Visit was given and must remain posted for 30 consecutive days. Appeal Rights provided.
