Based on observation, the licensee did not comply with the section cited above. LPA observed blankets hanging on the sides of a crib, while an infant was in the crib, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA reminded licensee cribs cannot have anything hanging on the side of them. Licensee removed blankets hanging on the sides of the crib during LPA's visit. Licensee stated they will not hang blankets on the side of cribs. Deficiency cleared during visit.
Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed an infant in a sleeping sack, while in a crib, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated infant's parents brought sleeping sack and requested infant to sleep in sleeping sack. LPA reminded licensee sleeping sacks cannot be used in any licensed childcare facility. Licensee stated they will not use sleep sacks. Licensee removed sleep sack from infant during LPA's visit. Deficiency cleared.
Based on interview and record review, the licensee did not comply with the section cited above as there is an unfigerprinted adult living in the home, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will get unfingerprinted adult fingerprinted and associated to the facility.
Based on (observation), the licensee did not comply with the section cited above in 2 areas in the home contained cleaning products availabe to children which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to remove items immediately and send a send a picture of products locked and removed by 5-16-22.
Based on (record review), the licensee did not comply with the section cited above in 2 staff and licensee did not have current Mandated Reporter Training on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees that she and staff will take triaing and send a copy to LPA by email no later than 5-20-22.
