Based on observation and interview, the licensee did not comply with the section cited above in that Staff 1 has not completed the required EMSA approved CPR/1st Aid which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Staff 1 will enroll in and completed the required EMSA Pediatric CPR and 1st Aid and a copy will be sent to LPA by POC due date 3/9/26

Based on record review, the licensee did not comply with the section cited above in the 1 out of 4 children's records reviewed did not have proof of immunization for Measles which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, imminization record will be requested from authroized representative of Child 3. Proof of required immunization will be sent to LPA by POC due date 3/9/26

Based on observation, the licensee did not comply with the section cited above in that door to bedroom 1 was slightly open but not enough to see 2 infants sleeping inside (photo taken) posing a potential risk to health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the door will be kept open and staff will be trained on Safe sleep regulations.

Based on observation, the licensee did not comply with the section cited above in 2 out of 2 infants sleepin in separate play yards have baby bottle inside the crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The bottles were removed form the play yard. Per licensee, the staff will be trained on Safe sleep regulations.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff present di not have proof of MMR vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee proof of MMR vaccine for Staff 1 and Staff 2 will be obtained from staff and sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above: Licensee did not have a current Mandated Reporter Training Certificate at the time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of completed mandated reporter training to LOA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above by not having an infant sleep log available for review for C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will read over the safe sleep regulations and provide documentation of an infant sleep log for C1 to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above: Licensee did not have proof of the fire extinguisher being purchased or serviced within the last year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of a purchased or serviced fire extinguisher to LPA via email by POC due date.
