Based on observations and interview, the licensee did not comply with the section cited above in 1 out of 1 2A10BC fire extinguisher in the home did not have service tag nor proof of purchase, licensee did not recall date of purchase which poses a potential health, safety or personal rights risk to persons in care.
Resolution: During this visit, the licensee ordered a new fire extingisher, Proof of delivery will be sent to LPA by 10/29/25
Based on observation, the licensee did not comply with the section cited above in that Child 1 was observed napping in play yard with baby blanket ant toy which poses a potential health, safety or personal rights risk to persons in care.
Resolution: During this visit, the provider removed lose items from inside the play yard and states they will ensure there are no lose items while infants nap in the play yard.
