Based on observation, the licensee did not comply with the section cited above in that child #1, 9 month old infant was observed in play yard with a stuffed animal and two toys which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licenee removed the stuffed aminals from the play yard and stated they will review the PIN 20-24-22

Based on record review, the licensee did not comply with the section cited above in that Licensee and staff have not renewed their mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the Mandater Reporter Training AB1207 compliant will be renewed and proof of completing will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in that Child #2 and #3 did not have immunization record which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee the immunization records for Child #1 and #2 will be provided to LPA via email by POC due dates.

Based on observation, the licensee did not comply with the section cited above not having at least one carbon monoxide detector in the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a carbon monoide detector and send proof to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above: Licensee and A1 did not have a valid current pediatric cardiopulmonary resuscitation card issued either by the American Red Cross or the American Heart Association, or by a training program that has been approved by the Emergency Medical Services Authority which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a pediatric cardiopulmonary resuscitation card issued either by the American Red Cross or the American Heart Association, or by a training program that has been approved by the Emergency Medical Services Authority to LPA via email by POC due date.
