Based on observation the licensee did not comply with the section cited above in that a slide located in play area is not sturdy which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the slide will be removed and a photo of the correction will be sent to LPA by POC due date
Based on observation, the licensee did not comply with the section cited above in the play yard observed with infant napping had blanket hung on side which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Staff removed the blanket during visit. Per licensee, they will review the safe sleep regulations with staff to make sure items are not placed on sides or inside the play yard while infants sleep.
Based on observation, the licensee did not comply with the section cited above by allowing two infants to sleep with blankets in the play yards which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed blankets from the play yards. Licensee will read the safe sleep regulations and provide a copy to assistants.
