HOFFMAN FAMILY CHILD CARE

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Adult #1, uncleared adult was present in the home observed providing care and supervison to children in care which poses an immediate health, safety or personal rights risk to persons in care. A $500 civil penalty was assessed during this visit.

Resolution: Adult #1 left the home during this inspection. Licensee stated they believed Adult 1 was cleared based on letter she recevied from the department on 2/28/25. Licensee stated they will appeal deficiency. Per licensee, Adult #1 will not return to home/facility until the background clearance is received.

Based on observation, the licensee did not comply with the section cited above in the Child 1 was napping in play yard with a blanket covering child's body which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff removed the blanket and stated they will ensure there is nothing in play yard while infant naps.

Based on observation, the licensee did not comply with the section cited above in that Child #2 was nappin on the ground without cot, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated child's cot was wet due to child wet the cot during nap, licnesee provided a cushion for child to nap in. Licensee stated they will purchase additional mats in case they are soiled. Proof of purchase will be she to LPA by 5/21/25

Based on observation, the licensee did not comply with the section cited above: LPA observed knives on the kitchen counter accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: A1 immediately removed the knives and placed them in an inaccessible area of the home.
