Based on observation, the licensee did not comply with the section cited above in that Child #1 napping in the play yard, there were lose items inside play yard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the clothing items from the side of play yard and removed lose items from inside. Per licensee, they will review Safe Sleep regulations with staff.

Based on observation, the licensee did not comply with the section cited above in Child #1 napping in the play yard, there were clothing hanging from side of play yard which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the clothing items from the side of play yard and removed lose items from inside. Per licensee, they will review Safe Sleep regulations with staff.

Based on interview and record review, the licensee did not comply with the section cited above in that Staff #1 does not have Immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee and Staff #1 proof of immunizations will be sent to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above in that there was a bottle of all purpose cleaner (Fabuloso) on the ground by sink located by door to the back patio accessible 5 children present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the cleaner and placed it in locked cabinet. Per licensee,

Based on observation, interview, record review, the licensee did not comply with the section cited above in that 5 out of 8 children present are under the age of 24 months which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During this inspection 3 out of 5 infants were picked up by parent/guardian. Per licensee, there are currently 5 infants enrolled and she will eithther disenroll 1 infant or change children's schedule to be in compliance with capacity. A written statement wil be provided to the department by 12/5/24

Based on record review, the licensee did not comply with the section cited above in 4 out of 5 children did not have LIC 995A in their files for LPA to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of completed LIC 995A to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above: LPA did not observed a current pediatric first aid card issued either by the American Red Cross or by a training program that has been approved by the Emergency Medical Services Authority which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a current pediatric first aid card issued either by the American Red Cross or by a training program that has been approved by the Emergency Medical Services Authority to LPA via email by POC due date.
