GONZALEZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in the there was 1 baby walker in the daycare room accessible to children in care which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the baby walker from the daycare and stated she will give the walker away. Per licensee, she will not have baby walkes in the daycare moving forward. The deficiency was corrected during visit.

Based on observation the licensee did not comply with the section cited above in that one out of 2 play yards had lose items inside playard while infant slep which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licesee removed lose items and items that were hanging from the sides or over the play yard during this visit. LPA provided Technical assistance and discussed safe sleep regulations includung PIN 20-24-CCP. The deficiency was corrected during visit.

Based on interview and record review, the licensee did not comply with the section cited above in that 1 out of 1 Adult - A1(volunteer) present did not have proof of immunization againstinfluenza, pertussis and measles, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Adult 1will provide proof of the required immunizations and will be mainitained on file at the facility. LPA will complete a future visit to review files.

Based on interview and record review, the licensee did not comply with the section cited above in that 4 out of 4 children present did not have immunizations available in the home license which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Child #1 - 4 will provide proof of the required immunizations and will be mainitained on file at the facility. LPA will complete a future visit to review files.

Based on interview and record review, the licensee did not comply with the section cited above in4 out of 4 children present did not have facility files including Emercency Information Form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, parents of Child #1 - 4 will provide proof of the completed LIC 700 and will be mainitained on file at the facility. LPA will complete a future visit to review files.

Based on interview and record review the licensee did not comply with the section cited above in that 1 out of 1 infant (age 10 months) did not have the Infant Sleep Plan (LIC 9227) completed and in the ile which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, parents of Child #1 will provide a completed Infant Sleep Plan (LIC 9227) and will be mainitained on file at the facility. LPA will complete a future visit to review files.
