Based on observation, the licensee did not comply with the section cited above in that Chiod 1 (a 5 month old infant) was napping in crib which was observed to have pads on 2 sides of crib which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The staff removed infant from crib immediately upon LPA arrival. Per licensee, the staff will be retrained on safe sleep regulations. The bumbers will be removed if it is to be used by Children in care. LPA provided technical assistance including information on the Departments Technical Support Program (TSP).
Based on observation, the licensee did not comply with the section cited above in that 2 out of the 2 infants napping in play yard/crib had a blanket while napping which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The staff removed blankets during this visit. Per licensee, the staff will be retrained on safe sleep regulations. LPA provided technical assistance including information on the Departments Technical Support Program (TSP).
Based on observation, the licensee did not comply with the section cited above in that Child 1 (a 5 month old infant) was napping in crib inside off limits bedroom with door closed which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The staff removed infant from bedroom immediately upon LPA arrival. Per licensee, the staff will be retrained on safe sleep regulations. LPA provided technical assistance including information on the Departments Technical Support Program (TSP).
Based on observation, the licensee did not comply with the section cited above in that cleaning compounds were located in kitchen cabinet under sink which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the cleaning compounds from the sink and placed them in locked garage. Per licensee, she will provided additional training to the cleaning persons to make sure the cleaning compunds are placed inaccesible to children in care.
Based on observation, interview, record review, the licensee did not comply with the section cited above in that Adult #1, a volunteer present during this inspection does not have proof of immunization against Influenza, pertiussis and Measles which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, proof of the required immunizations for Adult #1 will be sent to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in 1 out of 3 infants in care was observed in playard napping with baby blanket covering infant from the waist down which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Adult #1 removed blanket from play yard. Defeciency corrected during visit. Per licensee, the staff will be trained on Safe Sleep Regulations, a written statement confirming training will be sent to LPA by end of month 9/30/24
Based on interview and record review, the licensee did not comply with the section cited above in that 3 out of 3 infant files reviewed did not have Infant Sleep Plan, licensee confirmed that a log has not been created which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee created a sleep log during this visit and logged sleep times for 3 infants napping on this date. Per licensee, a completed sleep log will be sent to LPA on 9/20/24.
Based on record review, the licensee did not comply with the section cited above in that a Sleep Plan was not completed for Child #1, infant age 5 months, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the LIC 9227 Infant Sleep Plan will be completed and a copy sent to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in that chemicals observed in unlocked cabinet in the kitchen. Chemicals observed in unlocked cabinet in bathroom. Eazy off and Comet were found which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the chemicals from the cabinets and placed in garage. A safety lock was placed on the bathroom cabinet. Per licensee she will make sure to keep chemicals inaccessible to children in care. Deficiency corrected during visit.
Based on observation, the licensee did not comply with the section cited above in that 2 out of 3 infants in care had a pacifier with string attached to clothing which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the string and clip from pacifier and stated they will review and follow Safe Sleep regulations to ensure infants don't have attached pacifiers while napping.
Based on observation, the licensee did not comply with the section cited above in that Child 1, infant age 5 months did not have a fitted sheet while napping, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the play yard is provided by the parent, licensee will make sure there is a proper fitting sheet the next day that infant will be in care. Proof of correction will be set to LPA by photo by POC due date.
Based on observation and Interview, the licensee did not comply with the section cited above in the Child #1, age 14 months was observed sleeping in car seat during visit which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the child from the car seat. LPA provided Safe Sleep Technical Assistance, per licensee they will have parent remove children from car seat upon drop off at daycare, Licensee will provide safe sleep training to staff to make sure infants do not sleep in car seats.
