Based on observation and record review, the licensee did not comply with the section cited above in one out of one fire extinguishers which did not contain proof of date of purchase or a service tag which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain proof of date of purchase or will obtain a service tag and submit to LPA.

Based on observation, the licensee did not comply with the section cited above in that the kitchen undersink cabinet which contains cleaning products has a safet latch that is broken which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the cabinet latch will be fixed and a photo of the corrections will be sent to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants in care did not have a sleep log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the sleep log will be completed starting this date and proof of correction will be sent to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above due to can of raid was on a table in the kitchen accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the can of raid and placed it in the locked garage. Deficiency was corrected during visit.

Based on observation and interview, the licensee did not comply with the section cited above in 2 out of the 2 firearms in the facility are unsecured which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee secured one out of two firearms. Per licensee, they will locate the second firearms and turn both firarms in to be destroyed. Proof of correction will be sent to the department by POC due date (withing 24 hours)
