Based on record review, the licensee did not comply with the section cited above in one staff had no Mandated Reporter Training on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee Mandated Reporter is going to be completed and sent to LPA J.Duran via email on or before DEC 05,2025.

Based on observation, the licensee did not comply with the section cited above in that a carbon monoxide detector was not found during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of correction will be sent to LPA's email by POC due date

Based on observation, the licensee did not comply with the section cited above in that the latch on the kitchen cabinet where the cleaning products are kept is loose and not functioning which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of correction will be sent to LPA's email by POC due date

Based on observation and interview, the licensee did not comply with the section cited above in that Staff had placed an infant of 10 months in a car seat while they changed diapers of another child, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Staff removed Child #1 from the car seat immediately. Per licensee, Safe sleep regualtions will be reviewed with staff and the car seat will be kept out of the childcare area. A written plan will be sent to Child Care Licensing by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in 3 out of 3 children's files were missing, Child #1, #2 and #3 did not have facility fiels which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the children's fiels will be completed and a copy will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above due to facility roster is missing CHild #1, #2 and #3 information which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee the Facility roster will be updated and a copy will be sent to LPA via email by POC due date.

Based on LPA observation, the licensee did not comply with the section cited above due to the small wood fence that leads to the next door neighbor's yard has been remove. which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee completed a written statement of how they will supervise the children while the gate is replaced.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff records did not have the current Mandated Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the mandated reporter class will be completed and the certificate will be sent to LPA via email by POC due Date.
