Based on record review, the licensee did not comply with the section cited above in, Both Licensee and one staff had expired Mandated Reporter Training Certificate 04/10/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, both licensee and one staff mandated reporter training are going to complete and e-mail certificte to LPA J.Duran on or before 11/28/2025.

Based on observation and interview, the licensee did not comply with the section cited above,Expired Fire Extinguisher serviced on 04/10/2022 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licenee she will servie fire extinguisher and provide proof of service tag on or before 11/28/2025 via e mail to LPA J.Duran.

Based on observation and interview, the licensee did not comply with the section cited above in that Child #1 an 11 month old infant was sleeping in play yard with a small blanket annd toy which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the toy and balnket and stated she will make sure to follow safe sleep regulations. LPA provided Sleep

Based on record review, the licensee did not comply with the section cited above in 2 out of 6 children records reviewed - Child #2 and #3 were missing immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee they will provide proof of correction to licensing office by email by POC due date.

Based on record review, the licensee did not comply with the section cited above in 2 out of two staff did not have the required Mandaed Reporter Training Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will ensure all of the staff will complete the Mandated Reporter training by POC due date. A copy of the certificate will be sent to LPA by email.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 infant files reviewed did not have the Infant sleep log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will create an Infant sleep log for Child #1, #2 and #3 and send a copy via email to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above in 3 out of 4 children records reviewed - Child #1, #2 and #3 were missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee they will request and obtain the immunization records for Child #1, #2 and #3
