Based on LPAs observation the outdoor back yard has pieces of wood panels, a metal pole attached to a cement piece, unsanitary table, plastic giant blocks, 5 level stand up storage plastic rack (not anchored), plant inside a ripped base, 2 exposed barbeque grills, next to the grills there is a metal table (under the grills and metal table there are foam mat that appear to be unsanitary, on the right side of the house in a pass way there are stacked up tiles, and a bucket all accessible to children in care. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
