Resolution: Licensee will complete mandated reporter training and submit copy by POC due date.

Based on (observation), the licensee did not comply with the section cited above, sleep log is not maintained for infant in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will maintain 15 minute sleep log for infant in care and submit log by POC due date.

Based on (interview) , the licensee did not comply with the section cited above, disaster drill was not conducted at least every six months, the last drill was conducted in July 2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will conduct a disaster drill before due date and submit proof by POV due date.

Based on record review the licensee did not comply with the section cited above, file for child #1 was not availabe for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will provide enrollment file for child #1 by POC due date.

Based on (record review), the licensee did not comply with the section cited above, child #1 did not have immuniztions available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide copy of child#1 immunization records by due date.

Based on [(observation) the licensee did not comply with the section cited above Fire Extinguisher was purchased on 09/02/22 and has not been serviced yearly, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service fire extinguisher and submit proof of receipt by POC due date.

Based on [(observation) and (record review), the licensee did not comply with the section cited above, disaster drill was last conducted 03/09/23 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a disaster drill and submit log by POC due date.

Based on [record review], the licensee did not comply with the section cited above CPR First Aid is not EMSA approved for licensee, assistant Pediatric CPR First Aid has expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will complete Pediatric CPR and First Aid by POC due date.

Based on observation, the licensee did not comply with the section cited above in, LPA observed a baby walker in one of the rooms. Which poses/posed a potential health, safety or personal rights risk to persons in care

Resolution: Walker was removed during the visit, thrown in the trash.

Based on record review, the licensee did not comply with the section cited above Child #1 was missing proof of a safe sleep log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to start a safe sleep log and submit to LPA via email by POC date.

Based on LPAs observed there was an infant inside the baby carrier in one of the rooms. Per licensee her assistant, daughter was supervising the infant, LPA observed the infant to be alone in the bedroom with the door closed. Shortly after the daughter came out from the restroom.which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.

Based on record review Licensee, Staff 2 is missing proof of MMR and Tdap proof licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.

Based on record review Child 1 is missing proof of ndividual Infant Sleeping Plan LIC 9227 & Safe sleep log which posed a potential health, safety or personal rights risk to persons in care.licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.

Based on record review Licensee was missing proof of fire drill which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.

Based on observation LPA observe a ladder, wood panels, stove (not connected to a gas line) by the trash bins, Basketball court being held down with individual bricks that could be hazardous to children in care. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.

Based on LPA’s observation, the licensee did not comply with the section cited above in LPA observed an infant to be asleep and strapped in a baby carrier.

Resolution: Licensee removed the infant during the inspection.

Based on record review Staff 2 is missing proof of TB clearance licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she is going to submit proof to LPA via email by POC date.
