WASHINGTON, DENNIESHA FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above.The licensee stated that the assistant has not completed the Mandated Reporter Training. Licensee also mentioned that the assistant has been working with her since last year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to send proof of mandated reporter training by 07/03/25.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed under the bathroom sink hair products and a cleaning solution spray. The licensee stated she had to replace her her sink cabinet due to a flood and had not put a lock on it. LPA explained to the licensee the importance of having a lock on it, especially anything that says keep out of reach of children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items during the visit. Licensee stated she will put a lock on the cabinet and send proof to LPA by 05/29/24.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she has not done her mandate reporter training that expired on 07/21/23 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee will completed the mandated reporter training by 05/29/24 and send LPA proof of correction.

Based on observation and record review, the licensee did not comply with the section cited above in that the last fire drill conducted was on 02/13/2021. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall send documentation of a conducted fire drill to the Community Care Licensing (CCL) office. Proof of this correction shall be sent by the given due date.

Based on observation and record review, the licensee did not comply with the section cited above in that Child #1 and #2 do not have a signed and completed emergency information card on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated shall have the legal guardians of these children sign and complete the above information. Proof of this correction shall be sent by the given due date.

Based on observation and record review, the licensee did not comply with the section cited above in that Child #2 does not have a completed LIC 9227 form on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated shall have the legal guardians of Child #2 sign and complete the above form. Proof of this correction shall be sent by the given due date.
