Based on record review and interview, the licensee did not comply with the section cited above. Staff 2 did not have mandated reporter training certificate. Staff 2 did not have mandated reporter training. Licensee reported Staff 2 has been an assistant for a year, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff 2 will complete Mandated Reporter Training and will submit proof of completion by 02/21/2025 to the Department.

Based on observation and interview, the licensee did not comply with the section cited above. LPA Cabrera asked Licensee to open the unlocked top drawer of the kitchen cabinet. LPA observed small coins, white tablets (medicine), and small bag of marijuana. The Adult #4 confirmed to LPA that it was marijuana. At the time of the inspection, there were 2 infants and two small children present. The kitchen is an accessible area to the children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the coins and medicine. Licensee then gave the marijuana to Adult #4 to stored in a inaccessible area. POC Cleared at visit.

Based on interview and records, the licensee did not comply with the section cited above. During today's inspection, Licensee reported her adult son (Adult #3) turned 18 years on September 28, 2023, and he still lives in the home. Adult #3 is not background cleared with CCL, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure Adult #3 undergoes live scan processing in order to obtain background clearance. Licensee scheduled the live scan appointment for 12/01/2023 at 12:00 p.m. Licensee stated she will submit proof of live scan to the Fresno Community Licensing (CCL) office by 12/01/2023.

Based on observation and interview, the licensee did not comply with the section cited above. During the tour of the facility, LPA Cabrera observed Child #4 sleeping on the bed in Bedroom #2. Licensee reported the bedroom and bed belongs to her adult son. LPA asked if Licensee provided clean sheets for Child #4. Licensee answered no. Licensee did not provide safe and healthful accommodations, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a written plan indicating how she will provide safe and healthful sleeping accomodations for the children. Licensee will submit written plan to CCL by 12/08/2023.

Based on record review ,Employee rights and Child abuse statement missing from each employee file and copy of drivers license of staff that transport children were not provided at the time of the inspection. The licensee did not comply with the section cited above for 5 assistants, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have Licensing forms LIC 508, Employee rights, Drivers licensee for staff that transports children, and Child abuse statement for all assistants submtted by plan of corrrection date.

Resolution: Licensee agrees to make the back yard inaccessible until cleaned and made hazard free by POC date.

Resolution: Licensee agrees to clean and organize counter tops, clear walk ways and make bedrooms free of hazards.
