If Licensee's appointment falls after 07/15/2022, Licensee will submit verification of appointment.

Based on observation, the licensee did not comply with the section cited above. Licensee has ammunition stored in a case that does not have a locking mechanisim which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will store ammunition in a case that has a locking mechanism and submit verification to Fresno Community Care Licensing by 07/01/2022.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee was unable to provide documentation of Licensee checking on infants while they were sleeping which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 07/01/2022.
