Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee has been caring for infant since he was 4 months old and has not been doing the 15 minute log checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin documenting the 15 minute log checks effective today 06/05/24 and will send poof to LPA by 06/19/24.

Based on record review, the licensee did not comply with the section cited above in that the AB 1207 certificate provided to LPA was completed on 02/28/2019. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall re-new her certificate by the given POC due date. Proof of this correction shall be submitted to the Fresno CCL office.

Based on record review, the licensee did not comply with the section cited above in that the CPR/First Aid card provided to LPA expired on 02/15/2022. This poses as a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall re-new her CPR/First Aid card by the given due date. Proof of this correction shall be submitted to the Fresno CCL office.

Based on observation, the licensee did not comply with the section cited above in that LPA observed several children playing outside and observed several holes and dog feces in the backyard. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall remove the feces and fill the holes with dirt. Until the corrections are made, children shall not be utilizing the outdoor play area. Proof of this correction shall be submitted to the CCL office by the given due date.
