Resolution: The licensee will go through and clean the stains on the walls and carpets. The licensee will organize both sides of the house in the front yard.

Resolution: The Licensee will have every parents that had a missing LIC 627 form fill it out and file them in the cooresponding files.
Based on observation and record review, the licensee did not comply with the section cited above. During today's inspection, there was an adult providing care and supervision that did not have a California clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Staff member left the facility. Licensee contacted an assistant who has a California clearance to provide care and supervision.
Based on record review, the licensee did not comply with the section cited above. Staff member does not have immunization records on file verifying immunizaton against influenza, pertussis, and measles which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have staff member obtain immunization against influenza, pertussis, and measles. Licensee will submit immunization records to Fresno Community Care Licensing by 05/31/2022.
