Based on information obtained through record review, it was revealed that the facility had been out of ratio. This poses a potential risk to the health, safety, and personal rights of children in care.
Based on observations, the licensee did not comply with the section cited above, as LPAs observed that the business next door had wooden boards covering its front door. The licensee stated that on the morning of February 4, 2026, a car crashed into the front door of the neighboring business. The licensee did not report the incident to Licensing within 24 hours of its occurrence, which poses a potential health, safety, or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above LPAs observed that the telephone displayed notifications when calls were received; however, when pressing the talk button it did not connect the call which poses/posed a potential health, safety or personal rights risk to persons in care.
