Upon review of the sign in and sign out sheets, three parents signed in under the incorrect dates. This poses a potential health, safety or personal rights risk to persons in care.

Upon inspection of the kitchen area, the LPA observed two drawers that had items such as a screw driver, screws, E6000 glue, pointed scissors and a pointed meat thermometer in them. The LPA also observed sharp butcher knives on the bottom of the freezer which were accessible to children. This poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in all staff present had an approved Mandated Reporter Training current certifiacte which poses a potential health, safety or personal rights risk to persons in care.

(a)California’s Action Level for lead in water at Child Care Centers is 5ppb.(b) Testing results with fractional ppb readings of 0.5ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met by evidence by: Based on record review provided by Terraphase Engineeringl a result of (ALE) of 5.9 for outlet E which is out of compliance over the allowable ALE 5.5ppb.
