records review. During today's inspection it was found that although Staff #3 has obtained background clearance, she is not associated to the child care facility license as required. This poses a potential risk to the Health, Safety, or Personal Rights of children in care.
Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
(b)(1) Admission agreements shall specify the following: (1)Basic services. This requirement was not met by records review as staff did not have a procedure to documenting incident reports and dispersing to children's representatives. This is a potential health and safety risk to children in care.

Child's Records (d) All children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours. Records may be removed if necessary for copying. Removal of records shall be subject to the following requirements: (D) Records of any current illness or injury as specified in Section 101221(a)(9). (9) Record of any illness or injury requiring treatment by a physician or dentist and for which the center provided assistance to the child in meeting his/her necessary medical or dental needs. This requirement was not met as evidence by interviews and records review. This is a potential health and safety risk to children in care.
LPA's observations. Today, LPA observed Staff #2 and Staff #3 caring for a total of 21 children (Staff #2 is a teacher, Staff #3 is a teacher's aide). This poses a potential risk to the Health, Safety, or Personal Rights of children in care.
