(d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.

During the inspection of the outside play area LPA observed several areas that needed to be addressed for basic maintenance and upkeep prior to becoming a safety issue for the children in care. The areas discussed were the exposed metal on the play structure side steps that needs to be repainted/repaired and LPA observed several open cracks on the black top of the outdoor play area that can potentially be a tripping hazard. LPA advised that the sand box needs to be raked daily and ensured that it has a cover.

observations: LPA observed a section of the concrete walkway ramp is raised and there are cracks at the end of the ramp where a child tripped and sustained an injury. There are also cracks throughout the blacktop area which could pose as tripping hazards. This poses a potential risk to the health, safety or personal rights of children in care.
