MAYEN, SAIDY FAMILY CHILD CARE

Based on observation, interview, and record review, the licensee did not comply with the section cited above LPA reviewed 3 children’s files and observed that 2 files were missing the LIC 995 form which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA reviewed 3 children's file and 1 file did not have the emergency information form LIC700 and the LIC627 Emergency Consent Form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit documentation to the department by 10/31/25.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee handed LPA her fire drill log that was done in November 2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee to conduct a fire drill log and show proof to LPA by 07/19/24 via email.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed an infant inside a rocker chair asleep which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA explained to the licensee that she needed to immediately remove the infant from the rocker chair and place him inside the play pen or a cot. Assistant immediatley removed the infant from the rocker and placed him on a cot. Licensee stated the infant had just finished eating and fell asleep on the rocker chair. LPA explained to the licensee the safe sleep regulations. Licensee to submit a written statement to LPA that she has read and understands the safe sleep regulations by 08/01/24.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed in the kitchen a cabinet with cleaning compounds that were not locked or stored and were accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed them infront of the LPA and stored them in another cabinet that is high so children won't reach them. LPA explained to the licensee that she needs to ahve them in an inaccessible area at all times when children are present. Licensee understood. Licensee to write a written statement to LPA that she understands the regulations 102417(g)(4) by 08/01/24.

Based on record review, the licensee did not comply with the section cited above. Child #1, Child #2 and Child #3 did not have required immunizations on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof to Fresno Community Care Licensing of current immunizations for Child #1, Child #2 and Child #3 on or before 12/15/22.

Based on record reviews, the licensee did not comply with the section cited above. Licensee and Licensee's Assistant, Seidy Garcia did not have proof of Mandated Reporter Training certification, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and licensee's assistant will provide proof of updated Mandated Reporter certification to Fresno Community Care Licensing on or before 12/15/2022.
