Resolution: Licensee stated that she will sign up for the class and will send a picture of her registration status and will send apicture of her certificate when completed.

Based on observation and record review, the licensee did not comply with the section cited above. Licensee does not have a completed written emergency disaster plan which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete a written emergency disaster plan and submit a copy to Fresno Community Care Licensing by 12/23/2022.

Based on record review, the licensee did not comply with the section cited above. Licensee and staff completed mandated reporter training on 01/21/2020 and has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete mandated reporter training via www.mandatedreporterca.com and submit copy of certificate of completion to Fresno Community Care Licensing by 12/23/2022.

Based on record review, the licensee did not comply with the section cited above. Children's files were missing immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will request immunization records for the missing children and submit copies to Fresno Community Care Licensing by 12/23/2022.

Based on observation, the licensee did not comply with the section cited above back yard tools, a rake, hoe, exposed, propane tanks and other hazards accessible to children which poses a potential health, safety or pers onal rights risk to persons in care.

Resolution: Licensee has agreed to move items yard tools and propane tanks to an inaccessible location to children.
