Resolution: Licensee will replace swimming pool gate and notify Community Care Licensign by 06/23/2023.

Licensee and staff member has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and staff member will complete mandated reporter training and submit certificate of completion to Community Care Licensing by 06/09/2023.
