Based on observation, interview, and record review, the licensee did not comply with the section cited above. The infant's file does not have form LIC 9227 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will have the infant's parent completed form LIC 9227 and send proof to LPA by 03/29/24
Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed one of the children's files did not have immunizations on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get the immunizations from the parent and place it in the children's file and send proof to LPA by 03/29/24.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she does physically watch the infant when its asleep but doesn't document the 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will start documenting the 15 minute log checks and will submit proof to LPA by 03/29/24
Based on interview and record review, the licensee did not comply with the section cited above in that Child #1 did not have immunization records on file for review, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain copies of Child #1's immunization records and provide documenation of immunizations to CCL by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in that the last fire drill conducted and documented was April 25, 2022, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to conduct and document a fire drill with children in care by POC due date, and agreed to continue conducting fire drills at least once every six months as required. Licensee agreed to send proof of documentation to CCL by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in that Licensee does not document the 15 minute sleep checks required for Child #2 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she does conduct sleep checks, and agreed to start documenting the sleep checks for any infants 24 months and younger as required. Licensee agreed to send documenation of sleep checks for Child #2 to CCL by POC due date. LPA also gave Licensee a copy of safe sleep regulations for review, as well as template to use for documenting sleep checks.
Based on observation and record review, the licensee did not ensure fire drills were complete at least once every six months which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to conduct a fire drill this week and document the fire drill conducted. Licensee agreed to send proof to Community Care Licensing that a fire drill was conducted. Licensee also agreed to submit a written statement to Community Care Licensing indicating how Licensee will ensure that fire drills are conducted and logged as required by POC due date.
Based on record review, the licensee did not ensure Child #1's required licensing forms, including emergency authorization for medical care, was complete and signed by authorized representative. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to have parent of Child #1 complete all required licensing forms today, 04/25/2022. Licensee agreed to provide proof of completed forms to Community Care Licensing by POC due date.
Based on observation and record review, the licensee did not ensure mandated reporter training was completed as required every two years, with last training expiring in February 2021, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to complete mandated reporter training (www.mandatedreporterca.com) and submit proof of completion to Community Care Licensing by POC due date.
Based on LPA's observation, the licensee did not ensure hair developer containing hydrogen peroxide and other chemicals was kept out of reach of children which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the hair developer from underneath the bathroom sink and placed in an area inaccessible to children in care. Licensee agreed to submit a written statement to Community Care Licensing indicating how Licensee will ensure no hazardous items are accessible to children in care by POC due date.
