Based on observation and interview, the licensee did not comply with the section cited above. Licensee has not met the new pool safety requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will make sure she is in compliance and have a Self-closing, self-latching gate – with a key lockable device placed no lower than 60 inches above the ground, Maximum vertical clearance of two inches from the ground to the bottom of the enclosure, An approved safety pool cover that meets ASTM International Standard F1346-23 or an alarm that when placed in a swimming pool will sound upon detecting an entrance into the water, must meet ASTM International Standard F220, Life ring – diameter of 17 inches, approved by the US Coast Guard and Rescue pole with a body hook – minimum fixed length of 12 feet. Daily Inspection – Licensee shall perform a daily inspection of the safety features and equipment before opening the facility and maintain a log of the inspections to be provided to the Department upon request. LPA will do an in person case management visit to make sure licensee is in compliance.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. The licensee does not have LIC 9150 Additional Children in Care in 3 children's file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have each parent fill out form LIC 9150 and put it the children's file. Licensee will email LPA the forms signed and filled out by the parents by 03/19/24.

Resolution: Licensee stated she will have parents sign form LIC 282 and put in children's file. Licensee stated she will email LPA form LIC 282 with parent's signature on it by 03/19/24.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she physically checks on the infants while there asleep but does not log it which pose a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee stated she will start logging the times and date and if a child does not fall asleep Licensee will put on the safe sleep "no nap" on the date that infant did not nap. Licensee stated she will send LPA 2 weeks worth of safe sleep logs for all 3 infants by 03/19/24.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. The licensee does not have LIC 995 Parent's rights in 3 children's file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have each parent fill out form LIC 995 and put it the children's file. Licensee will email LPA the forms signed and filled out by the parents by 03/19/24.

Based on record review and interview, the licensee did not comply with the section cited above in that a child under 12 months in care did not have the required LIC 9227 Infant Sleeping Plan form on file for the child which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have parent of child submit form prior to returning to care. Licensee agreed to submit a written statement by POC due to date to CCL indicating Licensee understands the requirement to have all required Licensing forms completed prior to accepting children into care.

Based on interview and record review, the licensee did not comply with the section cited above in that three children did not have immunization records on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to get immunization records for all three children from parents and send to CCL by POC due date. Licensee also agreed to submit a written statement by POC due date acknowledging regulations require Licensee to obtain immunization documentation from parents before receiving children into care.

Based on observation, the licensee did not comply with the section cited above in that a spring on the trampoline was loose and needs to be replaced as well as the play structure slide appears to have been chewed has sharp edges which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to either replace or removed slide to ensure children do not have access to the sharp edges of the current slide and to replace or tighten trampoline spring by POC due date.
