Based on LPA observation, the licensee did not comply with the section cited above. Licensee failed to report changes to facility in making previously declared inaccessible room accessible to children in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to review licensing regulations relating to Alterations to Existing Buiding or Grounds, and provide the Department a written statement that they read and understood the regulations and will comply with licensing requirements.

Based on record review, the licensee did not comply with the section cited above. LPA observed two children's immunization documents missing dates when Measles vaccine was given. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to verify vaccination was given to both children and update immuization documents to show the date the Measles vaccine was given. Licensee is to provide the Department a copy of completed immunization documents.

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed broken drawer locks in Restroom 2 with accessible soap, shampoo, and toothpaste. LPA also observed off limits rooms with broken and unsecure locks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to install new drawer locks for bathroom drawers and new lever locks for door handles for all off limits rooms of the house.

Based on record review, the licensee did not comply with the section cited above. LPA observed immunization documents were missing for one assistant. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to obtain immunization documents from assistant and provide the Department a copy of immunization documents.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff files reviewed. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of measles & whooping cough immunization for Staff #2 and Staff #3 to the Fresno Community Care Licensing office, by 03/14/23.

Based on records review, the licensee did not comply with the section cited above as today, LPA found that the licensee's Mandated Reporter certification is not current as required. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete a renewal AB1207 certified Child Abuse Mandated Reporter course as required. This training can be accessed by logging on to the following website: mandatedreporterca.com Upon completion, licensee will submit a copy of certification to the Fresno Community Care Licensing Office, by 3/14/2023.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 children's files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit completed PM 286 forms for Child #2 and Child #3 to the Fresno Community Care Licensing office by 5/02/2022.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff files reviewed, as licensee was unable to provide proof of measles immunization for herself and Staff #1. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of measles immunzation for herself and Staff #1 to the Fresno Community Care Licensing Office by 5/02/2022
