GOMEZ, MARIA FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in upon arrival, LPA observed knives placed on the kitchen counter next to the sink, Two out of six children in care were tall enough to reach the knives, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately corrected the deficiency by removing the knives and securing them in a locked area, making them inaccessible to children.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 icensee was missing documentation for several days and a few months of the required 15-minute checks; which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure all future checks are completed and documented, and will provide a copy of the complete, Licensee will provide pictures of proof of completed 15-minute checks to LPA

Based on observation, nterview, and record review, the licensee did not comply with the section cited above in 2 out of 2 staff records reiveiwed did not have proof of Mandated Reporter Training completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she is currently register and plans to completed the training and will submitt proof of completion to LP on or before 07/10/2025.

Based on interview & record review, the licensee did not comply with the section cited above. Licensee stated that she does not maintain a file for herself or staff at the facility. This pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete a personnel file for all staff at the facility.

Based on interview & record review, the licensee did not comply with the section cited above. Licensee was unable to provide immunizations for self and staff at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain immunization records for herself and staff at the facilty by POC date.

Based on observation and interview, the licensee did not comply with the section cited above. Licensee stated that her adult son and his family (Wife-Adult Daughter and Minor Son) have recently moved into the home and will not be exiting the home until 7/8/2022. This poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed batteries and cleaning products available to children in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items from areas and will continue to keep them out of area accessible to children in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed weed killer in the back yard play area where children have access to. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all hazards to children in the back yard area by POC date. Licensee will ensure to not allow children in the backyard until the area has been cleared from hazardous items.

Based on interview & record review, the licensee and staff did not comply with the section cited above. Licensee stated that herself and staff have not completed mandated reporter training. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete training by POC date.

Based on record review, the licensee and staff did not comply with the section cited above. Licensee was unable to provide LPA with personnel files for self and staff. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete files for herself and staff. LPA will make a return visitl to verify completed documents.

Based on interview & record review, the licensee did not comply with the section cited above. Licensee stated that herself and staff currently do not have a valid CPR training certification. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete CPR training by POC date.

Based on interview & record review, the licensee did not comply with the section cited above. Licensee stated that approximetly the last fire drill at the facility was conducted about a year ago. Licensee was unable to provide any written documention of any drills conducted. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill and document it once every six months. Licensee will submit proof of completion by POC date.
