Based on observation, the licensee did not comply with the section cited above in upon arrival, LPA observed knives placed on the kitchen counter next to the sink, Two out of six children in care were tall enough to reach the knives, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately corrected the deficiency by removing the knives and securing them in a locked area, making them inaccessible to children.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 icensee was missing documentation for several days and a few months of the required 15-minute checks; which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will ensure all future checks are completed and documented, and will provide a copy of the complete, Licensee will provide pictures of proof of completed 15-minute checks to LPA

Resolution: Licensee stated she is currently register and plans to completed the training and will submitt proof of completion to LP on or before 07/10/2025.

Licensee stated that her adult son and his family (Wife-Adult Daughter and Minor Son) have recently moved into the home and will not be exiting the home until 7/8/2022.
