Based on observation, the licensee did not comply with the section cited above as LPA observed 3 infants sleeping in play pens with blankets. Swing observed in empty playpen which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will read infant safe sleep regulation 102425 and provided LPA with a statement of understanding. Licensee reported they will provide statement via e-mail on or before POC due date.

Based on record review, the licensee did not comply with the section cited above as Licensee stated their adult son is living in the home and is not criminally cleared. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will provide proof of adult son's fingerprint on or before POC due date.

Based on record review, the licensee did not comply with the section cited above in 4 of 4 infant files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will begin completing sleeping logs for all infants in care on an ongoing basis. Licensee stated she will submit proof of completed sleeping logs for Child # 1, Child #4 , Child #5, and Child #7 to the Fresno Community Care Licensing Office by 12/27/21.

Based on record review, the licensee did not comply with the section cited above in 1 of 3 staff files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of measles immunization, for herself, to the Fresno Community Care Licensing office by 12/27/21.

Based on record review, the licensee did not comply with the section cited above in 4 of 9 children's files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure all children's files contain record of immunization going forward. Licensee stated she will submit proof of immunization for Child #1, Child #3, Child #8, and Child #9 to the Fresno Community Care Licensing Office by 12/27/21.

Based on record review, the licensee did not comply with the section cited above in 3 of 3 staff files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Staff #1 & Staff #2 to complete an AB1207 certified Child Abuse Mandated Reporter course. This training can be accessed by logging on to the following website: mandatedreporterca.com. Upon completion, licensee will submit a copy of certification records to the Fresno Community Care Licensing Office, by 12/27/21.
