Based on LPA observation, the licensee did not comply with the section cited above. LPA observed excess clutter, excess toys, play equipment, personal items, and items not currently used by day care in all accessible areas of the home both inside and outside which could become an obstruction in the event of an emergency. These conditions pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to have all accessible areas of the home both inside and outside cleaned and organized, consolidate items, and have all excess clutter, personal items and items not regularly used by the day care removed or stored in an inaccessible area of the home.

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed stacked food items, stacked toys, stacked furniture, stacked storage containers, and other items not for use by the day care which could fall and injure a child. These conditions pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to remove all stacked items. Items not regularly used by the day care are to be removed or stored in an inaccessible area of the home.

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed the opening of the fireplace covered with a sheet of cardboard which could be easily removed by a child to gain entry. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to install a gate, door, or screen to prevent children from accessing fireplace. Licensee is to provide proof of installing barrier to the Department.

Resolution: Licensee is to complete new Mandated Reporter Training and provide a valid proof of completion certificate to the Department.

LPA Marquez observed the inside of the home has excessive household items: day-care, and activity supplies, food items, toys, and various miscellaneous items stacked about the home on kitchen table, kitchen counters and dining furniture. Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove any unneeded household items and refrain from stacking further household items on top home furniture. LPA to return for Plan of Correction inspection.
