Based on interview, the licensee did not comply with the section cited above as licensee reported an 18 year old adult living in the home is not fingerprint cleared which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated the 18 year old adult will obtain criminal record clearance. Licensee to submit reciept showing proof of live scan by POC due date.

Based on observation, the licensee did not comply with the section cited above as decribed in LIC809 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will make accessible areas free of clutter and organize toys, blankets and clothing.

Based on observation, the licensee did not comply with the section cited above as described in LIC 809 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will make areas inaccessible by installing baby gates and drawer locking devices or storing items out of reach of children.

Based on observation, the licensee did not comply with the section cited above as iinfant was observed crying for 20 minutes in dining room play yard, which poses a potential risk to the health safety and personal rights of children in care.

Resolution: Licensee immediately removed infant from play yard. Licensee stated she shall review safe sleep regulation and submitt a statement of understanding by POC due date.

Based on record review, the licensee did not comply with the section cited. Assistant was fingerprinted through LIVESCAN in October 2019. However, the clearance was closed and not completed. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee agreed to have her assistant re-printed through LIVESCAN marking the CCLD box on LIVESCAN form. Licensee will confirm her assistant receives a CA Criminal Record clearance and is associated to her facility prior to assistant returning to work in the facility.

Based on record review, the licensee did not comply with the section cited above in 4 of the 9 children present today are missing immunization records. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee stated she will obtain immunization records for child #1, #2, #8 and #9. Evidence of immunization records is to be submitted to the Fresno South licensing office by 7/7/2022.

Based on record review, the licensee did not comply with the section cited above. Both licensee and her assistant have not renew the required Mandated Reporter Training (MRT). Licensee MRT expired 5/6/2022 and assistant MRT expired 6/6/2022, which poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee stated she and her assistant will complete the MRT by 6/23/2022. Evidence of MRT is to be submitted to the Fresno South licensing office.
