The purpose of this inspection is in regard to a self-reported incident telephoned by Director Hernandez to the Officer of the day on 04/23/2024. On 04/22/2024, Director Hernandez reported that during a transition from recess to lunch two children were left outside for an estimate of four minutes. At that time 14 preschool children were present in preschool class. Teacher A and Teacher B were with children at this time. Teacher B took two children in the classroom and Teacher A stayed outside with the other 12 children. Teacher A gathered children to go inside. During the transition Teacher A was carrying water bottles when another child began to cry. Teacher A picked up crying child while holding the water bottles and told children to go inside. Teacher A and Teacher B did not do a head count when children were inside. During this time KAP KAY Staff walked down the ramp towards the infant classroom through the patio area and heard the children voices. KAP KAY Staff noticed the two children sitting at the table underneath the patio about six to eight feet from the preschool classroom entrance playing. KAP KAY staff notified Teacher A. Children were brought into the classroom. Teacher A and Director conducted a visual health check and children appeared to be in good health. Director stated that she spoke to Child’s 1 parent regarding the incident. Director was informed that Child 1 is currently going through a phase where the child hides. On 04/22/2024, Director addressed the supervision issue with Teacher A and Teacher B and discussed the importance of 100% child supervision and headcount. Director has devised a written plan and training on transition procedures from outside to inside. Director reported staff are provided with annual, quarterly training in active supervision, scan and count, zoning and teacher talk strategies. The last care and supervision of children training was conducted on 04/12/2024.
