Based on record review, the licensee did not comply with the section cited above in LPA observed that several staff had declined the MMR and Tdap vaccinations; which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in LPA observed that staff files were missing; LIC 508, LIC 9052, mandated reporter training certificates were all on line and not in files, CPR/1st aide cards are not in files. which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review and observation, one outlet at the facility contained a lead exceedance greater than 5.5 ppb. This poses a potential health, safety or personal rights risk to persons in care.
