Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee had two children in Bedroom #2 and one child in Bedroom #3. Per facility sketch Bedroom #2 and Bedroom #3 are inaccessible rooms and are not licensed for the children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the children from Bedroom #2 and Bedroom #3.

Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee stated she has not been recording the 15 minute checks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will begin recording the 15 minute checks for the infants and will provide LPA with copies by 10/03/2025.

Licensee completed last mandated reporter training on 08/28/2021 and has yet to complete renewal mandated reporter training every two years following the date on which she completed the mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete Mandated Reporter Certificate and will submit certificate to CCL by 08/21/2024. www.mandatedreporterca.com
