Type A citation - immediate risk to health, safety, or personal rights

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

Based on observation, the licensee did not comply with the section cited above as described in LIC 809, which poses/posed a potential health, safety or personal rights risk to persons in care.

(d) The surface of the outdoor activity space shall be maintained: (1) In a safe condition for the activities planned. This requirment was not met by evidenced of LPA observed that there is a 10 inch gap between the bottom of the wood chip box and the top this making a tripping hazard thus being a portential health, safety or persona rightsl risk to children in care.

Based on observation, the licensee did not comply with the section cited above as described in LIC 809, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation during the inspection, the licensee did not comply with the section cited above as the LPA observed three staff to be in the fenced outside playground with children, however all three staff were standing in a circle by the play structure talking and not supervising the children. This poses a potential health, safety or personal rights risk to children in care.
